The YMCA failed to properly register PCB Equipment, failed to inspect the PCB Transformers, and failed to maintain annual documents reports. The Albany YMCA failed to mark and properly store PCB contaminated electrical equipment and PCB Transformers in a Storage For Disposal Facility (SFD). The Albany YMCA failed to properly notify the EPA of their PCB Activity i.e. the SFD. The Albany YMCA failed to dispose of 3,689 kilograms (kgs) of PCB containing waste within one year from the removal from service date.
