Respondent failed to properly operate and maintain its facility; pipe leaks, corroded tanks, flow meters that were not calibrated, and the pretreatment flow meter was in the wrong location. Additionally, per DEC sampling, Respondent violated a Total Suspended Solids limitation at Outfall 002 in November 2009.
