## Summary

U.S. Environmental Protection Agency (U.S. EPA) Region 5 issued a Consent Agreement and Final Order (CAFO) under the Clean Air Act to Pollution Control Industries (PCI) for their facility located at 4343 Kennedy Avenue, East Chicago, Indiana. The CAFO requires that PCI pay a civil penalty of $25,000 and undertake a set of three SEPs at their facility. The penalty and SEP projects will resolve the Finding of Violation (FOV) and Notice of Violation issued to PCI on December 30, 1998. The alleged violations discovered in a 1998 inspection including violations of the Indians State Implementation Plan (SIP) and the National Emission Standards for Hazardous Air Pollutants for Benzene (Benzene NESHAP). The Source Reduction listed in items #20 and #22 are the result of a fire that occurred before corrective action could be achieved. An Administrative Penalty Order (APO) was issued concurrently to the CAFO.
