Administrative Order issued on August 20, 2010 for failure to conduct all standard monitoring during May 2009 according to its Standard Monitoring Plan (SMP), for total trihalomethanes (TTHM) and Haloacetic Acids (HAA5) as required by the Stage 2 Disinfectants and Disinfection Byproducts Rule (Stage 2 DBPR). Specifically, standard monitoring for TTHM and HAA5 was not conducted at two standard monitoring sites. The AO requires system to revise and submit an approvable standard monitoring plan (SMP). Collect a dual sample set for TTHM and for HAA5 at two standard monitoring sites during May 2011 according to your SMP, with all samples to be taken by May 2011. It also requires system to submit an Initial Distribution System Report and provide public notice for the above violations. Water system met the requirements of the AO by monitoring for total trihalomethanes (TTHM) and haloacetic acids (HAA5) according to a revised Stage 2 Disinfectants and Disinfection Byproducts Rule (Stage 2 DBPR) standard monitoring plan which was approved by EPA on September 1, 2010. Water system also submitted an Initial Distribution System Report on June 8, 2011. EPA issued Butler Water Department a Notification of IDSE Report Completeness on June 13, 2011. Finally, system provided public notice for the violations cited in the AO. A letter terminating the AO was issued on 6/21/2011.
