This Administrative Order is issued to Tamarack II and Joshua Townsley, co-owners of Fazooli's Family Italian, for exceeding the MCL of nitrate, failure to monitor for nitrate, failure to take a confirmation sample following the nitrate exceedances, failure to monitor for total coliform bacteria, failure to notify the public of the violations, and failure to report the violations to the State and EPA.
