This case addresses a facility located in Guaynabo, Puerto Rico which failed to file a Tier I/II form as required by EPCRA Section 312 to the EQB (the SERC), the LEPC and the Fire Department with jurisdiction over the facility for reporting years 2007, 2008 and 2009.
