Summary

CAFO issued 6/18/2002, assessing a penalty of $1,000. Respondent also agrees to complete a Compliance Incentive Project. Respondent failed to submit a completed emergency and hazardous chemical inventory form for lead 87 octane at its facility for calendar year 1998, 1999 and 2000, to the LEPC, the SERC and the appropriate fire department by March 1 of the year following the calendar year for which a report was required. Respondent did not submit, on or before October 17, 1987, or within three months after Respondent first became subject to OSHA's MSDS requirements, an MSDS or a list including the required information about lead 87 octane to the LEPC, the SERC and the appropriate fire department over the facility.
