This Combined Complaint and Consent Agreement and Final Order is issued to the Cedar City Regional Wastewater Treatment Plant for violations of the implementing regulations associated with the Prevention of Accidental Releases requirements and for failure to prepare and implement a risk management plan to detect and prevent or minimize accidental releases of such substances from the stationary source.
