On September 28, 2011, EPA Region 6 issued a Consent Agreement and Final Order (CAFO) under its authority pursuant to Section 1421 of the Safe Drinking Water Act to Mr. Clay S. Wilson, located in Pawhuska, Oklahoma. The CAFO was issued in response to violations of failing to submit annual operating reports for two injection wells for the period October 2009 through September 2010. The CAFO requires Mr. Wilson to pay a penalty of $800 and submit the required reports. Mr. Wilson must respond to EPA Region 6 within 10 days after receiving the CAFO.
