On December 12, 2011, EPA Region 6 issued a Compliance Agreement and Final Order (CAFO)under its authority pursuant to Section 1421 of the Safe Drinking Water Act to Performance Energy Resources, LLC, located in Barnsdall, Oklahoma. The CAFO was issued for operating an injection well without demonstrating its mechanical integrity and operating the well without authorization to inject from the EPA. The CAFO requires Performance Energy Resources, LLC to pay a penalty of $4,000. Performance Energy Resources, LLC must respond to EPA Region 6 within 30 days after the effective date odf the CAFO.
