This is an Administrative Penalty action pursuant to EPCRA 313 issued to Barrick Cortex, Inc. dba Cortez & Pipeline Gold Mine. The violations involve failure to file timely and/or correct Toxic Chemical Release Inventory Forms required under Section 313 for: antimony compounds, arsenic compounds, barium compounds, cadmium compounds, chromium compounds, cobalt compounds, copper compounds, cyanide compounds, nickel compounds, nitrate compounds, and thallium compounds in 2005; antimony compounds, arsenic compounds, barium compounds, chromium compounds, cobalt compounds, copper compounds, cyanide compounds, nickel compounds, and nitrate compounds in 2006; and antimony compounds, arsenic compounds, barium compounds, cadmium compounds, chromium compounds, cobalt compounds, copper compounds, cyanide compounds, lead compounds, mercury compounds, nickel compounds, nitrate compounds, and vanadium compounds in 2007. The violations were observed during a facility inspection on 8/13-14/2008. The CAFO assesses a penalty of one hundred fifty-eight thousand eight hundred ninety-two dollars ($158,892), and the facility agreed to conduct a three hundred forty thousand dollar ($340,000) Supplemental Environmental Project.
