This is a Notice of Violation, Administrative Order, and Reporting Requirement issued to Maritime International, Inc., Maritime Terminal Inc. d/b/a Connecticut Freezers, and Connecticut Freezers, Inc. for Clean Air Act General Duty Clause violations at a cold storage warehouse in New Haven, Connecticut. On May 25, 2011, an ammonia release occurred at the facility, resulting in evacuations. EPA inspected the facility and determined that the facility likely had violated the General Duty Clause of the Clean Air Act because, contrary to industry standards for ammonia refrigeration systems of this size (6,000 pounds of ammonia), the facility did not identify hazards at the facility using recognized hazard assessment techniques; maintain documentation about the refrigeration system that was sufficient for Respondents to adequately maintain and inspect the systemÂs equipment; have in place a sufficiently comprehensive preventative maintenance program; or appropriately label and mark refrigeration system components. The order and reporting requirement requires the Respondents to address these problems and conduct hazard analyses at three other cold storage facilities.
