On November 25, 2011, EPA Region 6 issued a Final Administrative Order (AO) under its authority pursuant to Section 1421 of the Safe Drinking Water Act to Jerry L. Clemishire located in Hominy, Oklahoma. The AO was issued in response to violation of maintaining an injection well in a manner which could allow contaminants to move into fresh groundwater resources. The AO requires Jerry L. Clemishire to complete appropriate corrective actions on the well to prevent contaminants from moving through the well into fresh groundwater resources within thirty (30) days of the effective date of the order.
