THIS PROPOSED COMPLIANCE ORDER, PENALTY COMPLAINT, AND NOTICE OF OPPORTUNITY FOR HEARING IS ISSUED TO SURFACE CREEK AUTO FOR OPERATING A CLASS V MOTOR VEHICLE WASTE DISPOSAL WELL AFTER THE JANUARY 1, 2007 BAN OF ALL MOTOR VEHICLE WASTE DISPOSAL WELLS. Facility was operating a banned Class V motor vehicle waste disposal well. Facility permanently closed the well within the timeframe of the order and so no penalty was collected.
