Water system failed to take annual nitrate samples and quarterly coliform bacteria samples, and notify the State and public of the violations. The AO was returned undeliverable. The owner said mail spills out of the mail slot and to send it to his home address. The AO was reissued on 11/7/00. The AO requires the system to sample for nitrate once and for coliform bacteria for four consecutive quarters. The system met the requirements of the AO to sample for nitrate in 2000, and also sampled during 2002 and 2002. The system sampled for total coliform bacteria quarterly except for the 2nd qtr./02, and the 2nd and 3rd qtrs./02, and eventually provided public notice for the violations cited in the AO, and public noticed for the violations that occurred after the AO was issued. On 5/2/03 R5 issued the system a letter terminating the AO.
