AO requires system to prepare a combined 1999 and 2000 CCR with 1998 and 1999 data, respectively. On 9/18/03 R5 issued a letter withdrawing the AO because WDNR confirmed that it inactivated the system as of 10/1/02 because it no longer serves water to a minimum of 25 persons or has at least 15 service connections.
