On December 21, 2012, EPA Region 1 issued a Notice of Violation, Administrative Order and Reporting Requirement to Millbrook Cold Storage, Inc. for failure to comply with Section 112(r) of the Clean Air Act in the handling of anhydrous ammonia at the company's Somerville, Massachusetts cold storage warehouse. Region 1 inspectors found several dangerous conditions at the facility during an inspection conducted on December 13, 2012, including corroding pipes and valves, breached vapor barriers on many pipes, ice coverings on some piping and valves that render access or inspection impossible, lack of a trained operator to run the refrigeration system, and inadequate labeling on pipes. As the Region finds that Millbrook has failed to analyze hazards associated with its management of anhydrous ammonia, the Administrative Order directs Millbrook to complete a process hazard analysis of the refrigeration within 30 days of the Order's effective date, and the Reporting Requirement requires the company to assess the amount of ammonia in the refrigeration system, among other things.
