On March 28, 2013, EPA Region 6 issued an Administrative Order (AO) under its authority pursuant to Section 1423 of the Safe Drinking Water Act to Hoyt Glenn, located in Osage County, Oklahoma. The AO was issued in response to the following violations:
1) failure to plug an injection well within one year after termination of injection operations; and (2) failure to maintain an injection well in a manner which would prevent fluids from moving through the well bore into underground sources of drinking water (USDWs). The AO requires Hoyt Glenn to either: (1) Plug the well or (2) submit a plan for its future use to the EPA, including proposed procedures to prevent contamination of USDWs. Hoyt Glenn must respond to EPA Region 6 within 30 days. For more information, see documents below.
