The Illinois Environmental Protection Agency (IEPA) conducted an inspection at South Holland Metal Finishing (SHMF) on March 16, 2010. During the inspection, the inspector noted several leaking containers of hazardous waste, and that SHMF was not using the correct EPA ID number on hazardous waste manifests destined to Envirite of Illinois. The IEPA referred the matter to the U.S. Environmental Protection Agency (EPA) for administrative enforcement on August 24, 2010. SHMF agreed to settle the matter with the EPA in a Consent Agreement and Final Order (CAFO). The CAFO required SHMF to pay a civil penalty of $20,764; to promptly address leaking containers of hazardous waste; and to use its correct EPA ID number on hazardous waste manifests.
