On 12/6/2011, Beelman Ready Mix LLC voluntarily disclosed violations of EPCRA 312 at five of its facilities. Each facility had stored hazardous chemicals in excess of the reporting thresholds and had not submitted Tier II forms for these chemicals to the SERC, LEPC and local fire departments with local jurisdiction over each facility for calendar year 2009.
