Respondent failed to maintain PCB Annual Document Logs, failed to mark the access to a PCB transformer in accordance with specifications, and stored combustible material within a PCB Transformer enclosure. The fence was subsequently marked, the combustible material removed from the enclosure and the PCB Annual Document Logs were reconstructed. Respondent will pay a penalty.
