Respondent exceeded the maximum contaminant level (MCL) for total trihalomethanes (TTHMs). Respondent determines combined filter effluent turbidity and the residual chlorine concentration entering the distribution system by conducting grab sampling. No evidence of public notification for the violations was available. The eye wash and showers in the area of chemicals failed to be working and the system failed to have day tanks for certain chemicals. The AO requires submission of 1) documentation related to the TTHM violation 2) an action plan for the correction of the violations identified and 3) public notification for failure to continuously monitor chlorine. Injunctive relief considers actions to address the TTHM MCL violation, correcting the failure to monitor and record using continuous monitoring equipment and address the showers and tanks.
