## Summary

On July 14, 2013, Region 10 filed a Consent Agreement and Final Order (CAFO) that resolves violations of the hazardous substances release reporting requirements of the Emergency Planning and Community Right-to-Know Act and the Comprehensive Environmental Response, Compensation and Liability Act by Star Ice & Fuel.  The CAFO addresses violations at Star Ice & Fuel’s former ice manufacturing facility in Fife, WA.  On September 7, 2011, a release of approximately 450 pounds of ammonia occurred.  Star Ice & Fuel failed to immediately notify the appropriate emergency response entities.  The company agreed to pay a penalty of $50,805.
