Kelly Gilby d/b/a John Gilby Production, the Respondent owned and operated an oil field production facility known as the Myrtle Green CTB located in Stephens County Oklahoma. On May 15, 2013 an EPA inspector observed that pollutants, specifically produced wastewater generated from oil production activities, had been discharged from the facility to waters of the United States withpout permit coverage.
