Based on the review of availble information, EPA determined that M & J Environmental Institute failed to provide notification following completion of renovator course, pursuant to 40 CFR 745.225(c)(14). In addition, M & J Environmental failed to provide to EPA notification of all renovator courses offered at least 7 buisness days prior to the start of the course, pursuant to 40 CFR 745.225(c)(13)(i). EPA and M & J Environmental Institute agreed settlement of this matter for a civil penalty of $400 utilizing an Expedited Settlement Agreement and Final Order.
