Alleged violations: (1) Failing to maintain required documents on site, (2) failing to
maintain a complete storm water pollution prevention plan, (3) failing to conduct
required self-inspections, and (4) failing to install and maintain best management
practices to minimize discharges of sediment and other pollutants into waters of the
United States.
