Respondent failed to submit a Periodic Report on Continued Compliance by June 30, 2013 as required by 403.12(e). Respondent exceeded its daily maximum limit of 24 mg/l with the result of 71 mg/l for the sample collected on November 22, 2013 and failed to submit pH results as required by 425.35(a) and certification as required by 403.12(b)(6).
