The Respondent had a Risk Management Plan, however, deficiencies were noted in the areas of Hazard Assessment, Process Safety Information, Process Hazard Analysis, Operating Procedures, Compliance Audits, Contractors, and Emergency Response. The violations were resolved through an Expedited Settlement Agreement.
