On May 13, 2014, Region 5 filed a Consent Agreement and Final Order (CAFO) with the Swan Corporation of Centralia, Illinois. Swan Corporation had failed to provide adequate employee training and had failed to maintain a contingency plan for the facility. The settlement requires Swan Corporation to provide training, maintain a contingency plan and pay an administrative penalty of $37,500.
