The system failed to correct or provide a plan to address significant deficiencies including evaluation of storage tanks and correction of significant deficiencies identified at the storage tanks and various significant deficiencies identified at specific treatment plants throughout the system. The system also failed to monitor at two sampling locations in 2011.
