Respondent failed to submit Periodic report on Continued Compliance by June 30, 2014 as required by 403.12(e) and failed to submit documentation to show that sample point # 2 was re-sampled and analyzed for oil and grease within 30 days after becoming aware of the violation for a sample collected on December 19, 2013 as required by 403.12(g)(2). In addition, Respondent failed to notify EPA in advance of any substantial change in volume or character of pollutants in their discharge as required by 403.12(j).
