Facility was not in compliance with all the elements of the Risk Managment Plan (RMP) progam which were identified during a week long inspection at the refinery from January 24 - 28, 2011. A complaint was issued on December 18, 2014 and the Respondent began to address the violations. However, on September 15, 2015, Hovensa declared bankruptcy. A compliance order was previously issued.
