Based on the findings of an inspection, EPA has compiled information related to several concerns such as:
Failure to maintain an updated facilities inventory and Operation and Maintenance (O & M) Plan; Failure to prevent tidal intrusion into the combined sewer system; Failure to prevent unauthorized releases of sewage onto public/private property.
