Respondent failed to submit a Periodic Report on Continued Compliance by June 30, 2014, December 31, 2013, June 30, 2013, December 31, 2012, June 30, 2012 and December 31, 2011 as required by 403.12(e) and exceeded the daily maximum and average monthly limits for Cr for a sample collected on Jan 11, 2013, for Cd, Cr, Cu, Pb, Ni, Ag, and Zn, for a sample collected on March 30, 2012 , for Cn for sample collected on Aug 28, 2012 and for Cn for sample collected on October 25, 2013. In addition, Respondent failed to notify EPA within 24 hours of becoming aware of violation, repeat sampling and analysis and submit results of repeat analysis to EPA.
