This ACO (along with the simultaneous issuance of a CAFO) resolves alleged violations identified in a NOV/FOV issued to BCM August 07, 2014. The alleged violations include violations of 40 C.F.R. Part 60, Subpart A, General Provisions; 40 C.F.R. Part 60, Subpart OOO, Standards of Performance for Nonmetallic Mineral Processing Plants; Title V; and the IL SIP. Specifically, the violations include failure to timely submit certain notifications, failure to submit certain other notifications, failure to implement good air pollution control practices to minimize emissions, failure to implement an operation program required by the IL SIP, and allowing fugitive dust from crossing the facility's property line. The resolution includes implementing an environmental management plan and practices to assure timely submittal of required reports, house keeping and maintenance practices, and general good air pollution control practices to minimize emissions. The facility is also required to apply for and obtain either a FESOP or a Title V permit.
