On May 13, 2015, a Consent Agreement and Final Order (CAFO) was issued to Saint Mary's Regional Medical Center in Enid, Oklahoma to resolve RCRA violations discovered during the review of data from its hazardous waste activities. The data revealed that on several occasions, Saint Mary's Regional Medical Center generated quantities of waste that triggered the large quantity generator requirements, and during those periods, they continued to notify and operate as a conditionally exempt small quantity generator of hazardous waste. The CAFO requires the payment of $113,300 and compliance with RCRA regulations.
