1. Failure to have the results of quarterly visual assessments of stormwater samples from each outfall and for not maintaining the documentation on site; 2. failure to include all the required information on the facility?s site map; and
3. failure to have documentation available to verify that the facility?s annual reports were submitted to EPA.
