Owner/Operator change requires to close the action before compliance. (dsa)On January 26, 2016, Respondent was served with the Final Administrative Order (Final AO) by Tulsa County Sheriff's Office. EPA issued Final AO under its authority pursuant to Section 1421 of the Safe Drinking Water Act to Steven & Shelly Cornelius, LLC, located in Collinsville, Oklahoma. The AO was issued because the company injected fluids through an injection well that did not have a current mechanical integrity test (MIT) and is maintaining the fluid level in two injection wells above the fluid level of the base of underground sources of drinking water (USDWs), which could allow contaminants to move through the wells into the USDWs. The AO requires Steven & Shelly Cornelius to cease injection and complete MIT on one well and complete corrective action on two wells to prevent fluids from moving through the wells into ground water resources. Steven & Shelly Cornelius must respond to EPA Region 6 within thirty days after the effective date of a Final Order.
