Respondent failed to implement a complete Leak Detection and Repair Detection (LDAR) Program using the procedures in NSPS Subpart VV- Leak Detection at Refineries as required by the Refinery MACT Subpart CC, failed to use appropriate equipment for LDAR as required by EPA Method 21, and failed to include all equipment subject to LDAR.
