On January 26, 2016, EPA Region 6 issued an Administrative Penalty Order (APO) and Consent Agreement and Final Order (CAFO) under its authority pursuant to Section 1421 of the Safe Drinking Water Act to Steven & Shelly Cornelius, LLC, located in Collinsville, Oklahoma. The APO / CAFO was issued because the company injected fluids on May 21, 2015, through an injection well that did not have a current mechanical integrity test (MIT). Respondent successfully demonstrated mechanical integrity on July 28, 2015.
