Respondent failed to submit nine (9) Periodic Reports on Continued Compliance as required by 40 C.F.R. 403.12(e) from December 2010 to December 2014 . Respondent also failed to sample for the parameters regulated as per the Pretreatment Regulations since at least December 2010 (except for June 16, 2014, June 22-23, 2011 and October 10, 2014) as required by 40 C.F.R. 403.12(b) and (e). Respondent has not sampled for the parameters regulated as per the Pretreatment Regulations during six (6) reporting periods.
