During CY 2011 and 2012 and 2013, Respondent had limestone, crystalline silica, Lafarge blended concrete. New Cem Slag, fly ash, diesel fuel, calcium chloride and Redi-Mix Concrete on the facility site at amounts greater than the minimum threshold level. Respondent failed to submit to the SERC, LEPC and Fire Department a completed Emergency and Hazardous Chemical Inventory Form including those substances.
