Failure to implement RMP requirements: 1) PHA did not address human factors; 2) Operating procedures did not include consequences of deviations; 3) Operating procedures were not certified annually; 4) Compliance audits were not conducted at a minimum 3 year frequency; and, 5) Facility emergency contact information was not accurate.
