Shannopin Materials, LLC and Mepco Intermediate Holdings, LLC (collectively, Respondents ) failed to immediately notify the National Response Center following an unpermitted release of 1,534.5 pounds of sodium hydroxide from the Facility located at Lat/Long: 39.771875/-79.935630, in Poland Mines, PA. The release was greater than the reportable quantity of 1,000 pounds and it occurred on March 16 or 17, 2015. Respondents agreed to pay a penalty of $8,356 to settle the matter and the EPA filed a consent agreement and final order to initiate and settle the claim.
