The City of West Haven failed to comply with its municipal separate storm sewer system ('MS4') Permit. In particular the City failed to update its Stormwater Management Plan, failed to submit Annual Reports, failed to conduct proper monitoring of its MS4 outfalls, failed to adequately map its MS4 system and failed to implement an adequate Illicit Discharge Detection Elimination plan.
