On October 27, 2015, an authorized EPA representative conducted a compliance inspection of Green Plains Holdings II, LLC to determine its compliance with the Risk Management Plan regulations and found that (1) Respondent failed to train or cause to be trained each employee in maintaining the on-going mechanical integrity of the process and (2) Respondent failed to inspect process piping at a frequency consistent with applicable manufacturers' recommendations and good engineering practices. An ESA was issued and a civil penalty of $1,080 was ordered.
