Region 5 inspectors performed a unannounced inspection at H.J. Mohr & Sons Co. in Oak Park, as a result of citizen complaints. During the inspection dust was observed crossing the property line, which is a violation of the Illinois State Implementation Plan, and a violation of the Facility's air pollution operating permit. The roads at the Facility were very dusty and the inspectors saw particulate matter emissions leave the site. Company representatives described the watering of the roads that occurs at the Facility, which was insufficient to meet the requirements of the Illinois SIP. Additionally and the Facility was unable to provided records of inspections of the plant, control systems equipment, and operations weekly as required by the Facility's operating permit. In addition, company representatives could not provide the Fugitive Particulate Operating Program required by the SIP during the inspection. When the Fugitive Particulate Operating Program was provided the next day via e-mail, it was missing many of the required elements including a map or diagram of the facility, locations of unloading and transporting operations with pollution control equipment, a detailed description of the best management practices utilized to achieve compliance, and the estimated frequency of application of dust suppressants by location of materials. After the inspection, EPA sent the Facility an information request and company representatives were unable to provide the fly ash receipt
