This CAFO resolves alleged violations of the Indiana SIP including failure to operate a flare to ensure 98 percent destruction efficiency, failure to comply with VOC and HAP emission limits, and failure to maintain records. This CAFO will be issued simultaneously with an Administrative Consent Order to return the facility into compliance.
