On September 28, 2016, a Consent Agreement and Final Order was issued to Texas Health Resources for failure to file a notification in 2011 - 2015; failure to meet the requirements of a small quantity generator; failure to perform adequate hazardous waste determination and failure to fully comply with the manifest requirements. Texas Health Resources will pay a penalty of $151,500.00, participate in a Supplemental Environment Project (SEP) with an estimated cost of $90,929 and establish compliance with the regulation.
