Respondent did not immediately notify NRC of ammonia release of July 1, 2012, a violation of CERCLA 103(a), and did not immediately notify Illinois SERC or LEPC, violations of EPCRA 304(a), and did not provide written follow-up notice to LEPC. Respondent had not submitted to SERC, LEPC and Addison Fire Department a completed Emergency and Hazardous Chemical Inventory Form including anhydrous ammonia for CY 2010 and CY 2011. Penalties of $4,433 for CERCLA violation and $14,037 for EPCRA violations were assessed. Company agreed to perform SEP in amount of $18,775 for purchasing emergency equipment and training for Addison Fire Department. The civil penalties have been paid.
